Worst LUT Management Screw-Up You've Seen On Set?

Hey all, Sam here. I'm on a commercial shoot right now with an AMIRA and the DIT is pretty on-point, but it got me thinking. I've always heard about the headaches poor LUT management can cause, especially if you're monitoring in the wrong color space. But I've never personally seen a catastrophic mistake that really tanks a production.

I'm curious to hear from those who've been around the block a bit more. What’s the most critical mistake you've witnessed firsthand regarding LUT management on set? And more importantly, how did that impact post-production? I'm trying to wrap my head around the 'worst case' to really understand the ripple effect beyond just a colorist's headache.
