On-Set LUT Management: Worst Mistakes & Post-Production Fallout?

Hey everyone, Diana here, working as an AD/Producer. I'm prepping for a new project, and we're looking at potentially shooting on an AMIRA with some older Cooke primes, and I want to get our monitoring and color workflow locked down from the start. I've been reviewing best practices, and the importance of correct color space monitoring really stands out. It got me thinking about potential pitfalls.

So, from your experience, what's the most critical mistake you've witnessed firsthand when it comes to LUT management on set? And more importantly, how did that error ultimately impact post-production? I'm talking anything from incorrect LUTs applied to recorders, monitors not calibrated, or even issues with the DIT's setup.
