LUT Management: What's the Worst Mistake You've Seen?

I'm Naomi, a production sound mixer, and something's been bugging me from a recent conversation. I heard about the pitfalls of monitoring in the wrong color space, and it got me thinking. While it doesn't directly impact my 4017, it absolutely affects the picture folks I work with.
I'm on a narrative short right now, mostly with an ALEXA Mini, and the DIT has been meticulous, but I've been on other shoots where things felt a bit looser. I try to stay informed on the visual side so I can better understand the whole workflow.
My question is: what's the most critical LUT management mistake you've personally witnessed on set, and how profoundly did it impact post-production? I'm genuinely curious about the real-world consequences beyond just 'it looked bad.'
