Sound Reports: Early Mistakes & Avoiding Post Headaches?

I'm supervising post on an indie feature right now, shot on a VENICE 2 with some great 4018s, and the sound reports coming in are... well, they leave a bit to be desired. It’s got me thinking back to my own early days with these reports and how much I probably messed up.
So, for all the seasoned pros here, thinking back to when you were starting out and first responsible for production sound reports, what was the most common mistake you personally made that led to significant headaches for the editors or sound mixers in post? Was it missing take numbers, unclear scene notes, or something else entirely? And, crucially, what's the one piece of advice you'd give a beginner today to avoid that specific pitfall? I’m genuinely curious to hear what caused the most trouble and how to best advise my team moving forward.
