Sound Reports on Chaotic Sets: Biggest Compromises?

As a Post-Production Supervisor, I've seen my fair share of sound reports, from pristine to... well, not so much. We all know the ideal; thorough notes, accurate timecode, detailed mic setups for every take. But then you get onto a fast-paced set, maybe with a smaller crew, or things just go sideways.
I was on a recent indie feature with an ALEXA Mini and some PavoTube II 30Xs, and the schedule was just brutal. We had a fantastic sound mixer, but they were literally running from setup to setup. We ended up with very minimal notes on mic choices (mostly just 'boom' or 'lav,' without specifying the 4018 vs. 6060 lavalier), and sometimes scene numbers were just broadly grouped. It definitely added some guesswork in editorial.
What's the biggest compromise you've had to make on a shoot regarding sound reporting due to a chaotic set? How did it impact post-production, or what did you learn from it for future projects? I'm genuinely curious about everyone's experiences.
